Chẩn đoán và điều trị lao sinh dục

Đại cương:

Lao sinh dục là bệnh lý xảy ra rất khó phát hiện có tỷ lệ khoảng 3% trong số các bệnh lao ngoài phổi

Triệu chứng lâm sàng:

Đa số trường hợp nhiễm vi trùng lao do lan tràn từ thận vào đường sinh dục.

Lao sinh dục nam:

50% trường hợp lao sinh dục nam cũng có biểu hiện lao thận. Tổn thương lao xảy ra nhiều nhất ở mào tinh và tuyến tiền liệt. Lao sinh dục nam thường không có các triệu chứng toàn thân. Trong trường hợp lao tiền liệt tuyến vi trùng lao có thể lây truyền qua tinh dịch.

Nhân cứng ở mào tinh hoàn, không đau và chủ mô của tinh hoàn bình thường.

Tràn dịch màng tinh hoàn.

Khám tiền liệt tuyến qua trực tràng và có thể thấy được nhân cứng ở một thùy bên, có thể sờ thấy túi tinh bị xơ cứng.

Lao sinh dục nữ:

Ngoài triệu chứng nhiễm lao chung (26%), các triệu chứng khác bao gồm: vô sinh (44%), đau khung chậu (25%), mất kinh (18%), xuất tiết âm đạo (4%).

Chẩn đoán:

Lao sinh dục nam:

Chẩn đoán chủ yếu dựa vào sinh thiết và cấy các sang thương lấy từ các mảng gồ.

Lao sinh dục nam được chia làm 3 nhóm:

Có bất thường trên phim X-quang hệ niệu hướng đến lao nhưng không phát hiện bất thường qua thăm khám trực tràng hay bìu (40%).

Có bất thường trên phim X-quang hệ niệu hướng đến lao kết hợp với sang thương ở các cơ quan sinh dục (50%).

X-quang hệ niệu bình thường nhưng có sang thương ở các cơ quan sinh dục(10%).

Xét nghiệm tinh dịch đồ có biểu hiện vô sinh.

Chọc dò dịch tinh hoàn.

Lao sinh dục nữ:

Nạo, sinh thiết nội mạc tử cung hay sinh thiết tai vòi tử cung để làm xét nghiệm mô bệnh học.

Chụp hình tử cung vòi trứng, nội soi ổ bụng, phết dịch âm đạo tìm vi trùng lao.

Chụp X-quang bụng có thể phát hiện vôi hóa hạch lympho vùng chậu hay vùng bụng. • Siêu âm có thể phát hiện các mảng (mass) phần phụ 2 bên với sự hiện diện vài nốt vôi nhỏ gợi ý nhiều đến tổn thương lao.

CT Scan cũng có thể giúp phát hiện sang thương lao sinh dục.

Mổ bụng thám sát hoặc nội soi ổ bụng đôi khi cũng cần thiết để xác định chẩn đoán.

Điều trị: 

Dùng thuốc kháng lao theo chương trình chống lao quốc gia: 2RHZE/4RHE.
